How to get access to your child’s MyChart

If you have your own MyChart account, you can request access to your child’s MyChart account by doing the following from the web site:

  • Log in to your account
  • Click Menu > My Record and then “Request Family Access”
  • Follow the prompts

If you don't have your own MyChart account and your child is under 18 years old, you can access MyChart by filling out an access request form for yourself and a child proxy access consent form for your child. When you submit the consent forms, you may need to provide documentation that you are the parent or guardian (e.g., a copy of your child’s birth certificate, a copy of your legal guardianship order).

Please submit the forms to Rush’s Health Information Management Office by emailing mychartrequest@rush.edu or faxing (312) 942-5549.

Protecting your child’s privacy

When your child turns 12, your MyChart access to your child’s record will change. At this point, you will only be able to access information about your child’s allergies, immunizations and growth chart.

This limited access is designed to protect the privacy and confidentiality of your child, in compliance with state and federal laws that restrict online access to medical information for any child 12 or older. When your child turns 18, they will be able to access their own MyChart record, and you can request access to it.
