Prepare For Your Visit

We work with you to submit all necessary forms to your insurance carrier.

Most carriers involve copayments which are due at time of service. We accept cash, checks or credit cards (Visa/MasterCard).

Workers compensation claims can be handled by bringing a claim from your employer or insurance company, a written acceptance of their financial responsibility, authorized by either by your employer or insurance company.


Patients are seen by appointment from 8 a.m. to 5 p.m. Monday through Friday. Appointments may be scheduled by calling 630-978-3800 between 7:30 a.m. and 5:30 p.m. Monday through Friday or by clicking below to submit an Appointment Request online:

We will always make an attempt to schedule your appointment in a most efficient manner. We make every effort to keep our appointment schedule on time but emergencies and other unforeseen events sometimes cause unavoidable delays. In such cases, we will do our best to keep you informed.

If you are unable to keep a scheduled appointment, please contact the office as soon as possible so that we may try to utilize that time for the care of other patients.

We share your concern about the increasing costs of medical care. Because statements and billing fees have become so expensive and in an effort to keep your medical costs down, PAYMENTS ARE DUE AT THE TIME OF SERVICE. Our fees are usual and customary for this area. Fees for surgery include the usual follow-up care, but do not include x-rays, casts or bandages. The fee for a first office visit or a new problem will be higher than the fee for a follow-up visit. We will be happy to discuss fees with you , and an estimate of fees for any surgical procedure will be given when requested.

Insurance & Medicare

We remind you that THE RESPONSIBILITY FOR PAYMENT RESTS WITH THE PATIENT BEING TREATED or the parent/guardian. For office visits we expect payment at the time of that office visit by cash, check, or credit card (Visa/MasterCard). If you have insurance coverage, we will submit the necessary insurance forms to your insurance carrier. We remind you that most insurance contracts, including Medicare, involve deductibles and/or percentage allowances with the result that the entire bill is seldom covered in full. Should we receive any payment which exceeds your balance due, the excess will be refunded to you. Patients who have Blue Cross Blue Shield (BCBS) and are employees of Presence Health or have health insurance coverage provided by a Presence Health employee: We do not provide surgical services at any Presence Hospitals. Therefore, you may incur additional costs if your surgical procedure is performed by one of our physicians. Prescription Refills

Prescription refills are issued during office hours only by phoning 630-978-3800. On weekends and evenings your medical record is not readily available and the appropriateness of ordering/reordering your medication is difficult to determine; therefore, refill requests will not be dealt with at these times. Anticipate your refill needs and do not wait until you are out of your medication to call. Should you experience any side effects to your medication, please stop taking it promptly and call us.
