Pre-employment Testing Requirements
After you have accepted an offer of employment, please call Rush Copley Healthcare Center, 1100 W. Veterans Parkway, Yorkville, at 630-466-3430 to schedule your pre-employment drug screen and physician appointment. Rush Copley Healthcare Center is open Monday through Friday from 8 a.m. to 5 p.m. and Saturday from 8 a.m. to noon.
The following pre-employment testing requirements will be completed during your appointment:
Drug Screen must be completed within
72 hours of accepting a job offer.
- Drug Screen
- Physical Exam - You must be able to perform the essential functions of the position accepted. If you are unable to perform the essential functions, an accommodation may be requested. If an accommodation is not reasonable, your offer of employment will be withdrawn.
- 2-step TB Skin Testing
- Measles (Rubaeola), Mumps, Rubella and Varicella immunity
- Tdap vaccination
- Hepatitis B (offered)
- N95 Mask Fit testing
Please bring the following to your pre-employment visit:
- Photo ID (required)
- Previous vaccination records or laboratory confirmation of immunity for the requirements above - If you cannot provide proof of immunity to these disease, your blood will be drawn and necessary vaccinations will be given during your visit. The Hepatitis B series will be offered to all employees, or a declination will be signed if proof of immunity cannot be provided.
- Previous TB records within one year - If you have had a positive TB test in the past, please bring any records of healthy evaluation or treatment for TB.
The requirements above are conditions of employment. An offer of employment may be rescinded if these requirements are not completed as requested.
Pre-employment testing may take up to 1.5 hours to complete during your initial visit and will require more than one visit to Rush Copley Healthcare Center in Yorkville. Please plan accordingly.
