This venture offers assistance for you or loved ones during recovery from an illness, surgery, injury or long-term medical problem, in the comfort of your own home.

Hours
Monday-Friday: 8:30 a.m. to 4:30 p.m.

Triage nurses are on-call evenings and weekends.

Assistance is available to you 24 hours a day, seven days a week including holidays.

Services and Programs

Community Alliance Home Health provides skilled and high-tech nursing care, comprehensive physical, occupational and speech therapies, wound care, IV therapy and medical social work services. The following programs are offered:

Skilled Nursing Care

Skilled nursing care includes, but is not limited to blood pressure monitoring, catheter care, medication management and wound and ostomy care. Home health care nurses will teach you techniques to help you gain independence.

Comprehensive Rehab Services – Physical, Occupational and Speech Therapy

Licensed therapists help you achieve optimal independent functioning, working with you to regain strength and movement through exercise. Our occupational therapists focus on activities promoting daily living tasks. Our speech therapist can teach you cognitive skills that will help you to become more independent and comfortable in making decisions necessary for general day-to-day living.

IV Therapy Program

Many patients require a variety of intravenous (IV) therapies to further their treatment and recovery. Our specially trained nurses administer IV therapies such as antibiotic, hydration, chemotherapy, pain management, hyperalimentation, blood transfusions, antiemetics and antianxiety medications.

Cardiac Program

Cardiac rehabilitation is important for recovery from a heart event or heart-related diagnosis. If you are unable to take part in an outpatient rehabilitation program, our cardiac care nurses provide services in your home. We use specifically developed disease management protocols and a multidisciplinary team approach. Working closely with physician and clinical specialists, our cardiac nurses monitor, educate and counsel you. Your care will include:

Cardiac Assessments
Disease management education
Medication monitoring and administering, including IV medications as order by a physician
Needs assessment for personal care assistance

Nutritional support and diet recommendations
Ongoing conferencing with your physician
Ongoing phone follow-up for evaluation, appointment and rehabilitation

Medical Social Workers

Social workers assist you and your family in identifying needs and options for additional services. They provide resource information and assistance so you can plan for and adapt to present and future care needs.

Certified Nursing Assistants Bath Visit

We offer a one-hour bath visit on a private pay basis to assist you with bathing and grooming. At your initial appointment, a Community Alliance Home Health registered nurse obtains your health history, assesses your current health status and coordinates care.

Venipuncture Program (Lab Services)

Our Venipuncture Program is the answer to having lab draws done in a timely basis within your home. A qualified, trained phlebotomist performs lab draws on a private pay basis.
