How do I make an appointment?
Atlas has three locations. To make an appointment call the office that is most convenient for you.
Please be sure to have your insurance information and physician referral handy when you call.
Are you a provider with my insurance company?
We participate with many, many insurance plans, and are always adding new plans. You can check with your insurance company to see if they have us listed as a provider, but might find that they have individual therapists listed instead of the clinic name. So, please check with our front office manager to verify if we are in your network.
What if I am "out of network"?
You can still see us for treatment even if you are out of network, and you can qualify for our prompt payment discount.
Will my insurance pay for Physical Therapy?
Most health insurance plans do cover PT, and you can find out if your plan has any specific co-pay by calling your insurance company and asking about your "physical therapy benefits".
How much do you charge for Physical Therapy treatment?
Our fee schedule is very reasonable, and the exact charge for treatment strictly depends on what procedure is required to treat each particular patient/diagnosis. Your Physical Therapist will be able to explain your recommended treatment plan to you at your first visit to our office.
