Now's the time to schedule your Sports or Back-to-School Physical!

Dr. McAfee provide complete physical exams that meet school and sports team requirements for children 16 years and older. Immunizations and lab tests are also available for an additional fee.

Children starting a new sport or beginning a new competitive season should also have a sports physical.  

Please bring along your school or sports physical form to the office when you visit. For your convenience we have included both a sports and school physical form that you can print off and bring to your appointment.

New State of Illinois Requirement for the 2012-2013 School Year.

This fall, students entering the sixth grade and the ninth grade will be required by the State of Illinois to be vaccinated with the TDaP vaccine to protect against Pertussis. Pertussis, also known as whooping cough, is highly contagious and has been on the rise in Illinois over the past few years.
