Financial Policy

Complete billing information will be provided at the time of your appointment.  Those patients who do not have insurance or are unable to provide adequate billing information are responsible for the total payment of their bills.

Co-payments, deductibles and any past due balances are to be paid in full at the time of service.  Our office accepts cash, checks, VISA and MasterCard.  Our office cannot bill you for co-payments.

Any payment arrangements, other than payment in full must be approved by our billing office prior to your appointment.  If prior arrangements are not made, you will be asked to reschedule your appointment.
