Referral Process

A referral is required for admission to the Rehabilitation Program at Rush-Copley.  If you or a loved one would like to seek admission, please call the admission coordinator at 630-978-4807. 

Medical professionals referring a patient will be asked send the following information via fax to (630) 978-6846:

  • face sheet
  • history and physical
  • physician progress notes and orders
  • therapy notes
  • nursing notes
  • current laboratory results
  • current list of medications
  • diagnostic test results

This information is important when considering the need of services.  The admission coordinator will review the information with Dennis Keane, M.D., medical director.  We will then call you to discuss admission or other alternatives.

Partners in Recovery

Designed for patients having Total Hip Replacement or Total Knee Replacement, Joint Endeavors offers information on preparing for surgery and knowing what to expect from your experience at Rush-Copley.

You will meet members of your care team, see any medical equipment that will be used, and learn what to expect during your hospital stay.  There will be plenty of time to address any other concerns you may have as well.

Joint Endeavors is held the first Monday of the month at 7 p.m., the second Saturday of the month at 10 a.m., and the third Friday of the month at 2 p.m.  We encourage all prospective patients to bring a significant other/family member as well so they are aware of what to expect as you recover from your surgery.
