Referral Process

A referral is required for admission to the Rehabilitation Program at Rush-Copley.  If you or a loved one would like to seek admission, please call the admission coordinator at 630-978-4807. 

Medical professionals referring a patient will be asked send the following information via fax to (630) 978-6846:

  • face sheet
  • history and physical
  • physician progress notes and orders
  • therapy notes
  • nursing notes
  • current laboratory results
  • current list of medications
  • diagnostic test results

This information is important when considering the need of services.  The admission coordinator will review the information with Dennis Keane, M.D., medical director.  We will then call you to discuss admission or other alternatives.

Joint Endeavors

Designed for patients having Total Hip Replacement or Total Knee Replacement, Joint Endeavors is a class that offers information on preparing for and recovering from joint replacement surgery.

You will meet members of your care team, see any medical equipment that will be used, and learn what to expect during your hospital stay.  There will be plenty of time to address any other concerns you may have as well.

We encourage all prospective patients to bring a significant other/family member as well so they are aware of what to expect as you recover from your surgery.
