Diabetes Education and Management

Diabetes is one of the most common and expensive chronic diseases.  Early diagnosis and treatment of diabetes is important to prevent or minimize complications.  Diabetes education and self-management training are essential for the individual to properly control their diabetes.  There is no cure for diabetes, but current treatment can give those with this disease good control over its effects.  Individuals with diabetes should be encouraged to take advantage of the medicines and technology available today to manage their diabetes.

Outpatient Services

Diabetes education at Rush-Copley's Heart Institute focuses on a collaborative approach to provide quality healthcare to those learning to cope and live with diabetes. A goal of diabetes education is to stay healthy and prevent complications by promoting better self-management behaviors among people with diabetes.  Our healthcare team works with the patient and family to adjust a treatment program that includes diet, exercise and/or medication.  Diabetes education involves a learning process that supports an individualized plan of care necessary to overcome treatment barriers and obstacles to self-care.

Comprehensive diabetes education consists of:

  • Assessment
  • Developing and implementing a plan of care specific to the standards
set by the  American Diabetes Association
  • Goal setting
  • Evaluation
  • Nutrition Counseling

Diabetes education appointments require a physician referral.  Referrals and orders for diabetes education are accepted from physicians, Medicaid, Medicare and healthcare insurance providers.

Diabetes education materials are available in English and Spanish.
