How can I obtain copies of a birth certificate?
Birth certificates can be obtained by contacting the Kane County Clerk's Office at 630-232-5950 or online at http://www.co.kane.il.us/COC/. The hospital does not issue certified birth certificates.

How do I get copies of medical records?
To obtain copies of medical records, please call 630-978-6786. An authorization form is required and can be found below. This form can be either hand-delivered or faxed to Medical Records. The fax number is 630-978-6858. Medical Records is located on the Garden Level of the medical center.

What are visiting hours at Rush-Copley?
As a place of healing, we believe that patients need a restful, quiet environment. Yet, we also recognize the very important role that loved ones play in their recovery. Therefore, with the exception of Women's Health, our general visiting hours are open. However, children under the age of six are not encouraged to visit, and an adult who will oversee their safety while here, must accompany those ages six through 12. Guests who may be ill, with symptoms such as coughing, fever, or sore throat, are kindly asked not to visit patients due to risk of spreading infection and compromising care. Patients in our Intensive Care Unit are permitted only 2 visitors at a time.
Women's Health has established the following guidelines in order to prevent exposing our newborn patients to contagious diseases:
- Visitation is available for children 12 years and older.
- Children under the age of 12 must be a sibling and provide a completed health screening form prior to entering the unit.
- Siblings who have completed a health screening form will be given a sticker to wear while visiting the unit. The form must be reviewed each time the child visits and a new sticker will be provided.

Rush-Copley Medical Center is located in the beautiful Fox Valley area, about 40 miles west of Chicago. The address is: 2000 Ogden Ave. Aurora, IL 60504. The center is four miles west of Rte. 59 and one mile east of Rte. 30

Promising extraordinary care is Rush-Copley's number one priority. If you would like to speak with someone about a recent experience, please contact our Patient Advocate at 630-978-4832. Rush-Copley also surveys it's patients periodically to measure our performance. We encourage you to complete the survey if you receive one and include any additional comments about caregivers and services.
